AUSTRALIAN REFUGEE AND MIGRANT CARE SERVICES LTD
Our service provides the bridge between the client and the service providers. We support, enable, and empower refugees to eventually manage their own health care and other needs. We remove the barriers preventing them from enjoying the mental and physical well-being they need to be able to confront the challenges of resettlement, and to thrive.
The service exists to support and facilitate clients to achieve improved quality of life, independence, improved health literacy and capacity, and health equality through wraparound case management support and direct services. Case management terms can vary according to the client’s needs, and may be short-term, ongoing, or episodic.
The target group includes refugees, and migrants with a refugee background. We currently provide services to clients who reside on Brisbane's Northside.
HEALTH CARE COORDINATION AND HEALTH ADVOCACY
This is done through the co-ordination of support using a flexibly tailored, well-developed array of services and supports in the community on a case management basis following a comprehensive needs assessment, and using a client-centred, strengths-based management plan with regular monitoring and review of agreed support plans to meet changing and complex needs. Regular interactions with the client ensure that they have a good understanding of the advice offered by health and service-providers, and assist them with meeting their obligations and responsibilities such as completing forms, providing information, attending appointments and complying with advice. Direct services include attendance at clients' appointments, home visits, transport where required due to disability, illness, and after surgery, reminder service, and advocacy and support in dealing with health care providers.
WRAP-AROUND SOCIAL SUPPORT AND ADVOCACY
Advocacy and support dealing with government, community, social, educational, and private services (e.g. pharmacies, landlords, property agents, utility providers, insurance companies, telecommunications services, and employers) with an emphasis on empowering clients to develop and maintain natural and social supports.
HEALTH AND LIFE SKILLS EDUCATION
Multilingual and culturally-appropriate educational presentations and materials which promote health and health literacy and/or address the social determinants of health e.g. domestic violence, sexual assault, gambling, scams.
SUPPORT FOR HEALTH PROFESSIONALS
We support and educate health and other professionals to develop the cross-cultural competence and the knowledge required to provide quality of care. We remove the extra burden on their resources, enabling them to remain profitable, and to concentrate on what they do best. Through our in-depth knowledge of the clients, their culture, history, community, and language, and the trust that we have earned from them, we empower clinicians and other service providers.
outreach eye clinics in partnership with qut health clinics
Twice a year, we run an outreach eye clinic in conjunction with QUT Health Clinics, where clients are able to have free eye tests performed by students under the supervision of their lecturer, together with a face-to-face interpreter. These clinics have been highly successful in providing the care needed by clients, in identifying many previously undiagnosed eye diseases, providing free spectacles to those who cannot afford them, and in providing the ideal opportunity for students to gain experience in a cross-cultural setting.
